Slips, trips and falls on the same level are the single most common cause of workplace injury in the UK — close to a third of all reported non-fatal injuries every year. Where they're a risk, the law requires you to assess them and put controls in place. The good news is that the fixes are usually simple and low-cost; the hard part is spotting every hazard before someone gets hurt. This tool does exactly that.

Walk an area, answer a guided checklist built on the HSE slips-and-trips framework, and the tool flags the hazards, scores the risk, and produces a recorded action plan you can file. It covers the full picture — floor contamination, surface condition, housekeeping and obstacles, cleaning regime, footwear, lighting, stairs and steps, and outdoor and weather hazards — with a specific control action for everything it flags.

It's a self-contained file that runs in any web browser: nothing to install, no account, no subscription, no internet needed once downloaded. Assess one area at a time and re-run it for every floor, staircase, kitchen or car park — yours to keep and use as often as you need.

What it does

  • Guides you through the eight HSE slip-and-trip hazard categories for any area
  • Flags the main slip cause (contamination) and the main trip cause (obstacles and housekeeping)
  • Gives a specific, practical control action for every hazard found
  • Scores the risk and raises the priority for public-access and higher-consequence areas
  • Lets you mark categories not applicable (e.g. no stairs in the area)
  • Produces a dated, printable assessment record with a 12-month review date
  • Points staff toward awareness training to help spot and clear hazards

How you get it

Frequently asked questions

Is this a substitute for a professional risk assessment?

No. It’s a structured aid that applies the HSE slip-and-trip framework to your answers, helping you identify hazards, decide controls and record your findings. The judgement of what’s adequate remains yours as the employer, and for complex or high-risk environments we’d recommend having your completed assessment reviewed by a competent health and safety professional.

What hazards does it cover?

All the main causes from the HSE framework: floor contamination (spills, water, grease — the leading slip cause), floor surface and condition, housekeeping and obstacles (trailing wires and clutter — the leading trip cause), cleaning regime, footwear, lighting, stairs and steps, and outdoor or weather hazards such as ice and rainwater. You can mark any category not applicable to the area you’re assessing.

Can I assess more than one area?

Yes. HSE guidance recommends assessing distinct areas separately, and the tool is built to be re-run for each — a warehouse floor, a staircase, a kitchen and a car park are all different assessments. Run it as many times as you need and keep a record for each.

Do I need to install anything or be online?

No. It’s a single self-contained file that runs in any modern web browser. Once downloaded it works completely offline, with no software, account or subscription.

Is it based on official guidance?

Yes. The checklist and controls follow Health and Safety Executive (HSE) guidance on slips and trips and the Workplace (Health, Safety and Welfare) Regulations 1992. It is not endorsed by or affiliated with the HSE or any other body — it’s an independent tool built on published guidance. You can find slips, trips and falls training courses here.
